Copies of Birth certificate, Social Security card, or passport must be attached.

Recent physical and immunization record must be included.

Permission trips must be filled out before your child can go on any trips or walks.

Any medication needed for your child must be accompanied with a doctor’s order and prescription.

Any person not on the pickup list will need to be identified before a child can be released in their care.
